We provide all-inclusive, coordinated care for older adults who need extra support to remain healthy, safe, and independent in the community.
The PACE Center at Mission Health Group delivers comprehensive medical care, rehabilitation, social services, and daily support under one unified program. Our team focuses on promoting independence, preventing avoidable hospital visits, and helping seniors maintain the highest possible quality of life. Care is personalized, ongoing, and built around the needs of each participant and their family.
PACE services include primary medical care, nursing care, medication management, therapy services, and social activities. Participants receive coordinated support such as physical, occupational, and speech therapy; chronic condition management; personal care assistance; and nutritional counseling. Transportation to the center and to medically necessary appointments is provided. Daily activities, social engagement, and caregiver support are built into the program to strengthen physical, cognitive, and emotional well-being. When specialty services are required, our team arranges and coordinates referrals to ensure seamless continuity.
PACE operates through an interdisciplinary care team consisting of physicians, nurses, therapists, social workers, dietitians, and activity coordinators. Each participant receives an individualized care plan based on a full assessment of medical, functional, and social needs. Care plans are reviewed frequently to adjust to changing conditions. Transportation, therapy, clinic visits, home support, and social services are coordinated by one central team to reduce fragmentation and improve safety. Families and caregivers are included in care planning to ensure clarity, support, and communication.
The PACE Center provides access to on-site laboratory testing through Mission Health Group’s CLIA-certified lab. Providers may order bloodwork, metabolic panels, infection screens, and other routine studies to support diagnosis and ongoing chronic condition management. Rapid tests for flu, COVID-19, and strep may also be performed on site. Results are typically available the same day, allowing the care team to adjust treatment quickly and accurately.
PACE Center visits include medical appointments, therapy sessions, social engagement activities, and wellness monitoring. Transportation is provided for all enrolled participants. New participants complete an initial evaluation that includes medical history, functional assessment, and care goals. Ongoing visits are scheduled based on each participant’s individualized plan of care, with frequent follow-up for those requiring closer management.
PACE is generally covered by Medicare, Medi-Cal, or both for eligible participants. Individuals who qualify for both typically have no out-of-pocket monthly premium. Those without Medi-Cal may have a monthly share of cost depending on income and resources. Before enrollment, our team conducts a full benefits review and explains coverage, services, and any potential costs. A Good Faith Estimate is available for individuals who request information about self-pay arrangements.
Services are delivered by licensed physicians, nurse practitioners, registered nurses, qualified therapists, and certified support staff. Interdisciplinary team meetings ensure every participant’s plan of care remains accurate, safe, and aligned with current needs. The PACE Center also serves as a supervised training environment for medical assisting, nursing, and allied health students rotating through Mission Health Group’s clinical education programs.
